Mandatory document in support of the application
You must submit a document in support of the application for financial assistance to contribute to the needs of a child who was born as a result of sexual assault. The document must show that the criminal offence committed against you has been reported.
What type of document can be submitted?
A report to a police force or another authority
For example, the supporting document may be:
- a police report
- a police officer's account of the events
- a written statement to the police force
- a charging document
- a judgement
Examples of legal entities: DYP, organizations in the Info Justice network, CSJ (including Rebâtir), Juripop, restorative or alternative justice organizations, legal clinics, AFPAD
A report to a victim support organization
For example, the supporting document may be:
- a copy of the organization's file
- a written statement to a service provider
- clinical notes
Examples of victim support organizations: shelters for women who are victims of domestic violence, CAVAC, CALACS, Native friendship centres, organizations that help people who are victims of sexual exploitation, organizations that help people who are victims of sexual abuse
A report to a health professional who is a member of a professional order
For example, the supporting document may be:
- a health assessment
- a copy of the medical record
Examples of health professionals who are members of a professional order: physicians, dentists, optometrists, pharmacists, nurses, social workers, psychologists and marriage and family therapists, sexologists, criminologists, psychoeducators
The attestation form of a service provider or professional completed by one of the above-mentioned people
If you cannot provide any of these documents in support of the application, you can submit an attestation form of a service provider or professional completed by a victim support organization, a police force, a legal professional or a health professional as mentioned above.
What information must the document contain?
In particular, the supporting document must contain the following information:
- Information about the victim:
- surname and first name
- full address
- telephone number
- IVAC file number, if known
- their social insurance number or health insurance number
- Information about the person completing the supporting document:
- surname and first name
- telephone number
- occupation
- employer
- Information about the criminal offence:
- description of the criminal offence
- date or period it took place
- where it took place
- Signature:
- signature of the service provider or professional
- date of signature
