If you received financial assistance from IVAC, you will receive a Relevé 5 tax slip, which must be included with your provincial tax return (Quebec income tax).
CNESST will produces and send the Relevé 5 tax slip on behalf of IVAC.
Tax slip are mailed between January 27 and February 7 every year. Please allow 10 business days to receive it.
